All students attending Spark! Discovery Preschool (Spark!) are required to have a General Health Appraisal within the last year. To ensure every child is compliant with this requirement,Spark! Discovery Preschool and the Early Childhood Department of SVVSD has invited Salud Family Health Centers (Salud) Mobile Unit to provide General Health Appraisals to any child who currently needs or will need one during the 2018/2019 school year.  If you would like your child to get their General Health Appraisal, you should know the following:● Salud Mobile Unit will provide General Health Appraisals on December 13th (parents MUST be present for these appointments)● Salud representatives will be at Spark! On December 6th during both morning and afternoon drop-off/pick-up times to answer any questions or concerns and help families fill out required paperwork. ● If parents/guardians have questions or concerns but cannot speak to a Salud representative on December 6th, they can email their questions to [email protected]● Parents/guardians need to fill out the attached Patient Registration Form and Child Registration Form prior to December 13th visit● Parents/guardians must sign-up for a designated time slot through Spark! Front office.Parents/guardians can call (720-652-7906) or sign-up in-person at drop off / pick up. Salud Family Health Centers accepts Medicaid, CHP+, and most private insurance plans. Salud also offers a sliding fee scale based on family size and income. Insurance enrollment assistance is available on December 6th. Allowing Salud to provide General Health Appraisals does not impact current medical homes. Salud is also happy to provide any follow-up medical care needed, but parents/guardians should follow-up with current medical home. Salud and Spark!
